What Each Of Our Health Care Workers Does For You:
Home Health Aides (HHA)
The Home Health Aide has received formal training in basic nursing skills and home management. The Home Health Aide is qualified to perform various services for the patient necessary to maintain their personal comfort and a clean, safe environment. The care and skills performed are provided with the supervision of a SelectCare Registered Nurse.
The specific job responsibilities of a HHA are the following:
A. Assist in the personal care of the patient including:
- Oral hygiene
- Bathing activities including shower, partial, or bed bath
- Toileting activities including the use of the urinal, bedpan, commode, assistance with ostomy appliances.
- Personal grooming activities including shampooing, skin care, shaving, perineal care
- Dressing activities
B. Performs basic nursing skills including:
- Exercises as prescribed by the physician or physical therapist
- Transfers from bed to chair
- Lifting, turning and positioning patient to prevent bed sores
- Transfers patients with the use of a Hoyer Lift
- Provides daily indwelling catheter care
- Recording vital signs as prescribed by the physician
- Records intake and output and weight
- Performs non-sterile dressing changes
- Assists diabetic patients in measuring blood glucose
- Cleans equipment used by the patient
- Keeps daily records of patient activities and observations
C. Provides household services in order to maintain a safe, clean environment including:
- Personal laundry of the patient
- Making and changing the patient’s bed daily
- Dusting and vacuuming the rooms used by the patient
- Dishwashing the patient’s utensils
- Tidying and wet mopping the kitchen and bathroom to ensure a clean and sanitary area
- Disposing of the patient’s garbage at the end of each shift
- Tidying the patient’s bedroom
- Shopping for the patient if no other arrangement is possible
D. Prepares and assists with the dietary requirements of the patient including:
- Preparation of the prescribed diet for the patient
- Assists with patient feeding when necessary
- Provides between meal nourishment
- Assists with menu planning and grocery shopping
E. Assists the patient in maintaining their daily activities and quality of life style including:
- Socializing with family and friends
- Accompany the patient to medical appointments, social activities, and outings as tolerated by the patient
- Assists the patient in the use of the telephone, writing letters
- Offers emotional support and reassurance
- Maintains an atmosphere of calm cheerfulness and productivity
- Provides activities geared to the patient’s interests
PERSONAL CARE AIDES (PCA)
The Personal Care Aide provides a safe, clean environment for the client and assists the client with personal hygiene under the supervision of a Registered Nurse.
A. Assist with the personal care of the client including:
- Good oral hygiene.
- Personal grooming activities.
- Bathing activities including shower, and sponge bath.
- Assist with shaving activities.
- Assist with skin care.
- Toileting activities.
- Dressing activities.
B. Provide household services in order to maintain a clean, safe environment including:
- Making and changing bed linens
- Dusting and vacuuming rooms
- Tidying and wet mopping the kitchen and bathroom areas
- Disposing of garbage daily
- Personal laundry of the client, wash, dry, and put away properly.
C. Prepares and assists with the dietary requirements of the client including:
- Grocery shopping and menu planning
- Preparation of the meals and assist with feeding when necessary
- Provide between meals nourishment
D. Assists the client in maintaining daily activities including:
- Provides activities geared to the client’s interests.
- Offers emotional support and reassurance
- Encourages client while she/he copes with the activities of daily living.
- Maintain an atmosphere of calm cheerfulness and productivity.
- Accompany client to the bank, hair dresser, doctor’s appointments, community activities, etc.
- Assist the client in completing personal business by the use of the telephone, writing letters.
REGISTERED NURSES (RN)
Registered Nurses are graduates of accredited schools or colleges and have passed the state board examination. They are licensed to practice independently, but work in conjunction with the client’s physician and SelectCare Director of Nursing.
Their patient care responsibilities includes:
- Assessing, implementing and evaluating the client’s nursing care.
- Performing nursing skills and procedures which are permitted by the Nurse Practice Act.
- Administering medications and treatments as prescribed by the client’s physician or dentist.
- Maintaining documentation of observations and nursing activities.
- Working directly with other health and social disciplines that are involved in the care of the client.
- Instructing family members and/or significant other individuals in the client care.
- Reporting significant findings or changes in your condition to the attending physician or the Director of Nurses.
- Maintaining a clean, safe environment in the client’s setting.
Registered Nurses are available for pre-pouring medication for the client as well as nursing visits and extended care shifts.
LICENSED PRACTICAL NURSES (LPN)
Licensed Practical Nurses are graduates of accredited vocational schools or practical nurse programs and have passed a state board of examination for licensure. They’re trained to performed a wide range of skills under the direction of a Registered Nurse and the client’s physician.
Their patient care responsibilities include:
- Participating in the assessing, planning, implementation and evaluation of the nursing care plan.
- Performing nursing skills and procedures which are permitted by the Nurse Practice Act.
- Assisting the Registered Nurse in the supervision of ancillary personal and family members.
- Administering medications and treatments as prescribed by the client’s physician or dentist as allowed by NYSNA guidelines.
- Providing direct skilled nursing and personal care activities.
- Maintaining a clean, safe environment in the client’s setting.
- Maintaining documentation of observations and nursing activities.
- Reporting significant findings or changes in the patients condition to the attending physician or the Director of Nurses immediately.
