The primary purpose of a free in-home care and safety assessment ensures that the client can be safely supported at home with appropriate home care staff.

Although our service team discusses the client’s care requests in detail over the phone, the RN assessment confirms that the client information is accurate and complete, and that the client understands the home care goals. Here’s an overview of what it entails:

- Identify Needs: The assessment aims to thoroughly evaluate the person’s physical, emotional, and mental well-being, as well as their ability to perform Activities of Daily Living (ADLs) like bathing, dressing, and eating, and Instrumental Activities of Daily Living (IADLs) such as managing medications, shopping, and preparing meals.
- Ensure Safety: A key part of the assessment involves a home safety check to identify potential hazards like fall risks and to suggest modifications for a safer living environment.
- Personalized Care Plan: The information gathered during the assessment is crucial for developing a customized care plan that addresses the individual’s specific needs, preferences, routines, and goals.

What to Expect During the Assessment:
- Information Gathering: Questions will be asked about the individual’s medical history, current health conditions, medications, any recent hospitalizations or falls, and their daily routines to help us ensure we build a safe plan of care.
- Physical and Functional Abilities: We will assess the person’s ability to perform daily tasks, their mobility and any physical limitations which helps us understand what specific tasks and assistance will have the greatest impact.
- Cognitive and Emotional Health: The assessor may inquire about memory, mood, social interaction, and any signs of anxiety or depression that aims to support our clients physically and emotionally.
- Home Environment Review: A walk-through of the home will be conducted to identify safety hazards and assess accessibility. Recommendations for home modifications or assistive devices might be made which is a part of our goal to create a safer home environment even when our caregivers are not present.
- Discussion of Preferences: It is important to understand the individual’s preferences, hobbies, interests, and cultural or dietary considerations to create a more person-centered care plan. Our team strives to make our clients comfortable and feel respected during every interaction with our team.

Together, your personal home health care team discusses your plan of care in detail to ensure your caregiver arrives to your home fully prepared, on time, and ready to make a difference in your life.
