Director of Nursing Gloria Fish is certified to complete Patient Review Instrument (PRI) Screens.  PRI Screens determine the patient’s appropriateness for placement in a skilled nursing facility (nursing homes) and is required by all facilities in New York and in several other states.

SelectCare will gather information from all members of the health care team and ensure that the document is complete and accurate.

The family, client or care giver will receive the original document for their admissions packet.
